Step 1: The psychotherapeutic consultation (Sprechstunde) and PTV 11
The aim: demonstrate urgency
The first step is to attend a psychotherapeutic consultation (psychotherapeutische Sprechstunde) with a therapist who works with statutory health insurance. The aim is to obtain the PTV 11 form, which certifies the need for and urgency of therapy.
How to proceed:
- Call 116117 or use the online appointment portal to request an appointment for a “Psychotherapeutische Sprechstunde”.
- Attend the appointment. There are two possible outcomes:
- Scenario 1: The practice offers you a therapy place. Great — you do not need this reimbursement process.
- Scenario 2: The practice cannot offer you treatment. Ask them to give you the completed PTV 11 form.
Key points to check on the PTV 11:
- It should include a provisional diagnosis and a recommendation for outpatient psychotherapy, for example behavioural therapy (“Verhaltenstherapie”).
- It should state that treatment is “required promptly” (“zeitnah erforderlich”). You will often also be given an urgency code.
- It should state that therapy cannot be provided by that same practice.

Step 2: Document an unsuccessful therapist search
The aim: a record of your search
Your health insurer needs evidence that you have tried unsuccessfully to find a therapy place with a statutory-health-insurance psychotherapist. You should therefore keep a record of all calls and emails.
How to proceed:
- Search for psychotherapists in your area who accept statutory health insurance, for example through 116117, the websites of the Associations of Statutory Health Insurance Physicians (Kassenärztliche Vereinigungen), or your federal state’s chamber of psychotherapists.
- Contact at least 5–10 practices by phone or email. Ask explicitly for a therapy place (Therapieplatz), not only an initial appointment (Erstgespräch).
- Record every contact attempt. Note the date, time, name of the practice, contact person and outcome, for example “waiting list > 6 months”, “not accepting new patients” or “unreachable by phone”.
A waiting time of more than three months is considered unreasonable and may support an application under the cost reimbursement process.

Step 3: Medical certificate of need

In addition to the PTV 11, many health insurers ask for a short certificate from your GP or a specialist, such as a psychiatrist, confirming that psychotherapy is urgent and cannot be postponed.
How to proceed:
- Make an appointment with your doctor.
- Explain your situation and ask for a certificate of need or urgency (Notwendigkeits- oder Dringlichkeitsbescheinigung) for your psychotherapy application.
- The certificate should confirm that, from a medical perspective, a long wait for therapy is not reasonable.

Step 4: Submit the application to your health insurer
The aim: submit all documentation
You can now assemble your application for cost reimbursement. We recommend sending it to your health insurer by registered post (Einschreiben).
Required documents:
- Application letter: your formal request for reimbursement (see the German template below).
- PTV 11 form from the psychotherapeutic consultation (see step 1).
- Record of your unsuccessful therapist search (see step 2).
- Medical certificate of urgency (see step 3).
- Certificate from the private practice: confirmation from us (or another private practice) that treatment can begin promptly. We will provide this after our first contact.

Step 5: What to do if your application is rejected
The aim: submit an objection (Widerspruch)
A rejection is frustrating, but it is often not the final word. It is essential to submit a written objection. The health insurer is required to offer you an alternative. If it does not, your prospects of success are good.
